In short
According to Ukraine's Ministry of Justice (24 June 2026), a repeat certificate can be obtained by the person concerned, parents or guardians, a representative with a notarial power of attorney or a lawyer — through a consulate, via Diia with an electronic signature, or at any civil registry / administrative service centre in Ukraine.
Who can apply
The person the record concerns; parents or other legal guardians of a child; a representative under a notarised power of attorney; a lawyer.
Route 1: a Ukrainian consulate
You apply at a Ukrainian consular office in Poland. Convenient if you have no electronic signature, but allow for queues and document shipping time.
Route 2: Diia
An online application on diia.gov.ua with a qualified electronic signature. The document is delivered by Ukrposhta within the region — so someone in Ukraine has to collect and forward it.
Route 3: a representative in Ukraine
You sign a power of attorney at a Polish notary for a relative or lawyer in Ukraine and get a sworn translation into Ukrainian. The representative collects the certificate at any civil registry office or administrative service centre and sends it to you.
Next steps in Poland
- For a registry office, voivodeship office or university — a sworn translation into Polish
- Apostille — usually not needed for Poland (1993 PL–UA agreement, Art. 15) but needed for third countries
- Keep the original: some institutions compare it when you file
